Primary Behavioral Health LLC is required by law to maintain the privacy of your Protected Health Information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

We may use and disclose your PHI to bill and collect payment for services from you, an insurance company, or a third party.
Healthcare Operations
We may use and disclose your PHI for activities necessary to operate our practice, including quality assessment, staff training, and licensing.
As Required by Law
We will disclose PHI when required by federal, state, or local law, including reports to public health authorities and in response to court orders.
Uses and Disclosures Requiring Authorization
Most uses and disclosures of psychotherapy notes, uses for marketing, and sales of PHI require your written authorization. You may revoke authorization at any time in writing.

Complaints
You may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.
Changes to This Notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have about you, as well as any information we receive in the future.
